| Dani’s House of Pizza Win a Date with Mr. Dani Contest Rules 1) Each contestant may enter the contest by filling out the online entry form available on Dani's House of Pizza Web site or by coming into the store in person. Online, you may send us a link to your MySpace or Facebook page with your picture, or after clicking "Submit," you may send us a photo of yourself in JPEG, GIF, or BMP format via e-mail, No risque pictures, please. 2) Each contestant must write in Fifty (50) words or less why they would like a date with Mr. Dani. 3) Only females 25 years of age or older are eligible to enter. No purchase necessary. The decision of the judges is final. Contest is not open to employees of Dani's House of Pizza or their families. 4) Only one entry per contestant is allowed. No profanity or vulgar language is allowed in the entry form or it will be disqualified. The winner will receive the following prizes: A $250.00 gift card to the store of their choice - Dinner and drinks for 2, with Mr. Dani, at a restaurant in the Meatpacking District in Manhattan - Round-trip transportation. The date and time of the dinner engagement shall be selected after the winner is determined and shall be at such a date and time that the winner is available to attend. The presentation of the gift card shall be the date of the dinner engagement. 5) The winner must sign a release form, prior to the date, certifying that she is twenty-five (25) years of age or older and absolving Dani’s House of Pizza, its owner, employees, associates, and affiliates from any liability for any unforeseen events that may occur on the date, that are not directly caused by Dani’s House of Pizza, its owner, employees, associates, or affiliates. Dani's House of Pizza's liability under any circumstances shall solely be limited to providing the stated prizes to the winner or the monetary value thereof. 6) By entering the contest, contestants agree that Dani’s House of Pizza may use their name, image, and likeness in any promotional materials, including usage on their Web site. 7) Not responsible for typographical errors or omissions.
